Annual employee premium = monthly gross premium less monthly employer contribution, floored at zero, × 12. Add user-modeled covered cost sharing, bounded by the entered in-network out-of-pocket maximum.

Hypothetical worked example

A $500 premium less $200 contribution costs $3,600 annually; $2,000 modeled covered cost sharing gives $5,600.
